The years before aesthetic surgery
Dr. Saman was trained in both reconstructive and aesthetic facial surgery, but some of the most formative years of his career were spent treating substantially more complex problems: major facial trauma, cancer defects, facial paralysis, and advanced head and neck disease.
These were operations in which the goal was often not simply to reconstruct appearance. Treatment could require restoring swallowing, speech, airway, facial movement, and other essential functions.
Anatomy might be distorted by trauma, removed by cancer, scarred by previous surgery, or simply different from what had been anticipated. Some decisions had to be made urgently. The operation had to adapt to the problem in front of the surgeon.

Leading complex reconstructive care
As Director of Head and Neck Oncologic and Reconstructive Surgery, Dr. Saman led a multidisciplinary team involved in the care of patients with complex cancer, trauma, and reconstructive problems.
The work extended beyond the operating room. It included complicated postoperative care, critically ill patients in intensive care, coordination across multiple specialties, and responsibility for decisions when outcomes depended on much more than the technical performance of an operation.

Why that experience still matters
Reconstructive surgery taught him to think beyond a planned operation: to understand anatomy deeply, anticipate problems, recognize when something is not proceeding normally, and adapt when the anatomy in front of him differs from what was expected.
Dr. Saman believes those years made him a better aesthetic surgeon than he would have been had his experience been limited to elective aesthetic surgery alone.

Rhinoplasty, particularly revision surgery, frequently involves anatomy changed by previous operations, scar tissue, structural weakness, airway problems, grafts, or unexpected findings.
Dr. Saman does not apply one philosophy to every nose. Preservation techniques are used when appropriate. Structural techniques are used when necessary. The anatomy determines the operation.
Facial rejuvenation

Deep facial and neck surgery requires familiarity with facial nerves, vascular anatomy, salivary structures, muscle, fascia, and the deeper layers of the neck.
These are anatomical territories Dr. Saman worked in long before his practice became exclusively aesthetic. That experience continues to inform his approach to facial rejuvenation, advanced neck surgery, deeper anatomical structures, and complex surgical situations.

A deliberately focused practice
Over time, Dr. Saman’s practice has become increasingly concentrated around rhinoplasty, revision rhinoplasty, and facial rejuvenation.
The focus is intentional. These are operations in which anatomy, judgment, technique, and accumulated experience matter enormously, and they remain the areas to which he devotes most of his surgical work.

Dr. Saman teaches rhinoplasty and facial rejuvenation through invited lectures, surgical courses, international meetings, and visiting-surgeon education.
More than 45 established surgeons have traveled to New York to observe and train with him.
In 2025 he was selected as one of six surgeons worldwide to perform live ultrasonic preservation rhinoplasty at the Montpellier Live Rhinoplasty Course in France, and was the only American surgeon on the live-surgery faculty.

Dr. Saman wrote The Smart Guide to Rhinoplasty because patients making permanent decisions about their faces deserve to understand what they are agreeing to.
It was not written to convince people to have surgery. It explains what rhinoplasty can change, what it cannot, how surgeons think about the operation, and what patients should understand before choosing both a procedure and a surgeon.
Sometimes better information strengthens the decision to proceed. Sometimes it does the opposite. Both are valuable outcomes.

Dr. Saman has spoken and written about the increasingly young age at which children are being introduced to adult skincare, anti-aging products, and beauty standards.
His concern extends beyond whether a particular ingredient belongs on young skin. It is also about what happens when children are taught to see normal features as problems requiring correction.
His work has addressed the Sephora Kids phenomenon, the physical effects of inappropriate skincare products, and the larger cultural question of introducing appearance-related insecurity during childhood.
